US Healthcare System Test 1 Study Guide Purpose of this class Firstly medical schools would expect you to know about the US healthcare system because you will be working within the system and secondly any citizen of this country deserves this information for self knowledge Why is it important to know the administrative stuff Because as a healthcare provider you need to know because they will be accountable for this part as well US Healthcare system vs Other Healthcare W 5 First World Countries The US healthcare system isn t like other systems because Not really a system because it is not as developed compared to other 1st world countries Popuation Because it is not centralized Fewer physicians per person different goal for different ppl companies people care Service vs Care Care meaning someone will care what happens to you but service means the seller tries to make the most out of the business Most believe US is more service based Customer vs Patient Patient information Less power for consumer Power balance Care delivery No control in quality of care Service delivery Care not guaranteed Return xit negotiation Lawsuits Reasons why US healthcare is a Business Examples of Industry Hollywood Oil and gas real estate What is in common for all Pro t Business What does this system look like EquipmentHMO s 490ctorsHealthcareWgt Patients 7 Service Customer Corrupt system practitioners have to give into because it all part of the system Parting from the system could put their reputation into trouble Know the de nition of a system Now compare that to the US healthcare system Private Practice Examples of Private Practice Hospitals Clinics out patient pharmacies rehab centers home health hospice care nursing care nursing homes longterm care facility optometrists dentists mental facilities psychiatric facilities System interconnected parts that work together Problem All these facilities are not interconnected to work together Ex When pt is discharged from longterm care her ndings are unnececary till she returns to the hospital What does this result in Redundancy for the repetitive charges Not cost effective Frustrating Lack of goa pt has to make the executive decision without the right tools and knowledge pt falls into gaps INHIBITION OF CARE Heath De neib State of physical and mental well being 0 Carry your own activities 0 Physical status 0 Not being in pain Determined by socioeconomic status and mental health Compromised health states Real def Physical and mental wellbeing at complete eve Nah not true Example Health of some1 in DenverCO compared to someone in New York City NY Possible determinants Genetics Physica environment natural environment Built environment Socia environment cultural beliefs socioeconomic status lifestyle in uences Risk taking behaviors Occupation Support system or nah But problem is so many varying factors but people in both these locations are paying the same for health insurance costs Another thing to keep in mind Negative health related Behaviors like smoking seat belt goggles fast food So who is working on Prevention Nobody cares No one cares about longterm care for himself or herself They hand it over to the providers but the providers are also in it for the short term healing period Insurance Who gets money from their premiums Car insurance vs health insurance just because you didn t see a doc doesn t mean you re healthy Why prevention is not a focus Because no one is gaining anything out of it No commercial gain Too forward in time Opposite focus More patients to see if mo ppl sick Preventionlt Medical Model prmary v secondary prescription based society tertiary why deter customers Court system All healthcare employees work for the govt Ministry of health Ministry of edu National health service Fragmented Vendors What s the role of pharmaceutical companies Make money off of what ppl are suffering Changing costs as demand goes up is not illegeal but if don t illegally many CEO s got arrested for manipulation of prices SOpeople don t re ll prescription because of COST Power and In uence is the upper hand of poitics businesscooperation media DME Durable Medical Equipment Know what equipment falls into this category wheel chairs beds MRI cat scans mammography machines etc Disposable Industry gloves syringes napkins band aids tubing bags Keeps prices up because it s a necessity Stocks can be bout into these companies If company pro ts makes dividends and stock buyer proliferate Also grow in market because of stock buyers pharm companies Not your regular supply and demand curve As demand increases price of the product also goes up Emergent Care Why do people misuse emergency dept f emergency dept is close than their healthcare provider By doing this people are taking away time from the real emergency s ua ons The hours are exible How does this affect the dept as a whole Because of the increase usage of the emergency dept a process called redistribution occurs This therefore increases the cost across all areas in the hospital What could be a potential solution A possible solution is to divide the urgent care facility from the non urgent care which is the clinical care The people who try to take advantage of this when they don t need it must be charged additional charges But it is not fair because the providers would actually recommend these visitations of they are charged with such a hefty price so is that fair trying to capitalize money from the wrong people Sliding Scale A payment based on the income of the patient or the family Nonplrofit hospital vs pro t hospital V Rotary Mission Based ther orgs V like any other hospital but tax don t get cut because they prove that they are donation based or offer some kind of aid Corporations Large corporations with more than 50 employees must provide health insurance for all their employees At this point it becomes important for corporations to keep their employees healthy so they won t cut too much from the insurance budget How they do it Workshops seminars 5ks etc All of ces have a health center employed by the company to ensure any needs to employees Provide health medication and psychological exam of other employees Home Health Sitter Home health and sitter are both different For home health the patient would need the actual prescription of a healthcare professional to tell them but for sitter it is not part of them medical system although they could attain one for themselves if needed Home health Usuay include RNPTOTCAN certi ed assistant nurse a big eld because its booming and worker works in hisher own autonomy employs RN s doctor recommended so insured by insurance part of the healthcare system because these providers are medically trained Sitter Private payment Mostly in geriatric population Job includes preparing them for the day and ensuring housekeeping not part of the healthcare facility
