Spring Semester 2018
INSTRUCTOR: DR. Gray Generalized Anxiety Disorder Women more than men;     prevalence     peaked around middle age ● Women have historically to worry about kids more ● “Prevalence”: Won’t get worse and worse, people who develop disorder,  greater number of diagnosis at middle age. So much worry about self and  others, career, family, kids, etc. DSM­5 diagnostic criteria  ● Excessive anxiety and worry ○ At least 6 months ● Difficult to control Anxiety and worry are associated with >or= to 3 Restlessness
Easily fatigued
Difficulty concentrating or mind going blank
Irritability
Muscle tension
Sleep disturbance
background image GAD Treatment Reductions and management ­ not eradication ● CBT ○ Relaxation; cognitive monitoring and restructuring; exposure  where relevant/posible ■ Give people ability and capacity to understand how they  generate extreme appraisals and perceive them as truth ■ Experience it and realize they can handle it and that it wasn’t as bad as they imagined ○ Acceptance/mindfulness v. attempts to control emotions ■ Develop skill to focus on present moment (anxiety is a focus  and freak out over the future) ■ Develop understanding that thoughts do not necessarily mean  truth ■ Initial experience is usual tolerable, it is the extreme efforts we  go through to avoid experiencing, make it worse ● Attempts to reduce anxiety will make it worse ○ Alcohol, drug use, etc. Meds ­ benzodiazepines  (be careful, very careful) ○ Traditional “antidepressants” may be helpful & less risk than benzos ■ SSRI don’t give a sense of euphoria, no sense of an extreme  “high” but still affects NTs Benzos  ■ Quickly dampen functioning, reduce  GABA, reduce  respiration, heart rate ■ Become increasingly tolerant & become dependent and abused ● Doesn’t fix issue w/ chronic anxiety Social Anxiety Disorder Intense anxiety about social situation where one might be judged or scrutinized
background image ● Humiliation/embarrassment ­ excessive/disproportionate ○ Normal human experience, but fear is excessive  ● Extreme avoidance ○ Ex. Avoid Social interactions, being observed (e.g., eating),  performing ● Nearly 1 in 10 ● Negative reinforcement & treatment implications ○ Anytime there is something aversive or unpleasant, anything I can do  to avoid it is a negative reinforcement, but adding to that worry ■ Ex. Fear of eating, if I do something to terminate fear (leave  place), now reinforced that I will avoid any and all behaviors  that avoid fear ○ Exposure hierarchy, cognitive challenge ■ Instead of avoidance, slowly do things that are anxiety  provoking in an increasing intensity ■ Exposure is ideal when possible Fear of people watching you eat 1. Worried→ making a mess, spilling, way you eat, food in teeth
2. Avoid→ Messy foods (taco, chili dog, spaghetti), Dates, 
Restaurants,  Make list of wha worries/ what you want to avoid for a week
­rate each item from 0­100 of level of worry about it (create Hierarchy) 
Make person do lower level anxiety thing and gradually escalate  ­ Eat a cookie @ 4pm Fri. → Eat it @ a busy time → Eat a full  meal → … ­ Continue moving up hierarchy of fear Obsessive-Compulsive Disorder (OCD)
