A career in this field of pharmacy started when a group of pharmacist in the nursing home environment focused on providing medications and health services to the elderly. Today, you can find them in varieties of settings such as in the psychiatric hospitals, assisted living facilities, in-home and community based care, hospice programs, and the like. The consultant pharmacy practice is a special pharmacy discipline that actually started in the provision of pharmacy services to various health care settings including nursing facilities and long term care environments. As a matter of fact, this practice has originated more than three decades ago, but today the definition and job descriptions have significantly changed.
