Anus, Rectum, Prostate (HA Concepts Notes) Anus Assessment:  Obtain permission and explain procedure  Position patient: lateral decubitus, knee-chest, or standing Inspection:  Check for: lesions, warts, hemorrhoids, fissure, abscess, fistulas, polyps Palpation  Palpation for lesions or external hemorrhoids  Consider a Papanicolaou smear of anus if any history of anal intercourse o Sphincter tone  Internal  External o Walls  Surface, consistency  Presence of stool o Internal hemorrhoids o Perform stool for occult blood Colorectal Cancer Screening nd  Colorectal cancer (CRC) is the 2 leading cancer killer in the US  Is preventable  CDC states if everyone age 50 and older had regular screening, at least 60% of deaths from colon cancer could be avoided  Screen for life: colorectal cancer action campaign o Screening tests at a glance  High-sensitivity fecal occult blood testing (FOBT)  Sigmoidoscopy  Colonoscopy o May prevent by identifying precancerous lesions or find CRC early for more effective treatment  Risk factors o Over 50 years of age o Family history  Currently second leading cancer killer in the U.S. o However, it should not be; if everyone age 50 or older had regular screening tests, 60% deaths from this cancer could be avoided o Screening identifies precancerous polyps so they can be removed before they become cancer o Screening can also find CRC early, when treatment can be effective o CRC is most often found in people age 50 and older o Older you get the higher your risk; both men and women get CRC  CRC screening tests include o Fecal occult blood test (FOBT) o Flexible sigmoidoscopy o Combination of FOBT and flexible sigmoidoscopy o Colonoscopy o Double-contrast barium enema Checklist 2  Inspect anus and perineal area  Inspect during Valsalva maneuver  Palpate anal canal and rectum on all adults  Test stool for occult blood Prostate  Walnut size (3 cm X 4 cm X 2.5 cm) and shaped gland with distinct medial groove located below the anterior wall of rectum  Best examined with patient standing and bending over the exam table  Avoid in patients with acute myocardial infarction or patient in neutropenia state Palpation  Consistency: rubbery, smooth, non-tender  Lateral and anterior-posterior  Masses (nodular) 3
