In an acid-base reaction, acid donates a proton while base accepts a proton. Thus, an acid-base reaction involves the transfer of the proton from an acid to the base (an electron rich species). Loss of a proton from an acid produces conjugate base while acceptance of proton by the base produces conjugate acid. Thus, the acid-base reaction results in the formation of conjugate acid and base.
